Inguinal Hernia in Childhood


Abstract
This study is a retrospective analysis of 356 cases of inguinal hernia in children under the age of fifteen years who were operated at the Department of Surgery, Han Gang Sacred Heart Hospital, College of Medicine, Hallym University during the
period of
9 years and 6 months from January, 1985 to June, 1994.
@ES the results were as follows.
@EN 1) The male to female ratio was 4.15:1 and 63.7% was admitted for opertion at the age between 1 to 6 years.
2) the inguinal hernia was discovered at the age between 1 to 6 years in 61.5% and less than 1 year in 27.2%.
3) In 56.5%, the operation was conducted within 6 months after discovering the hernia, but was delayed for over 6 months in 43.5%.
4) The incidence of right hernia was 1.85 times higher than the left one.
5) The typeof hernia was indirect in all of cases and the sliding hernias were noted in 4 cases(1.1%).
6) Reducible inguinal or scrotal mass was observed in all of the patients with uncomplicated hernia(86.5%) and the irreducible one was in all the cases of complicated, incarcerated or strangulated hernia(13.5%). Among the complicated
hernia(13.5%),
incarcerated hernia was in 13.2% and strangulated hernia in 0.3%, and the incidence of incarcerated hernia was 1.68 times higher in male(14.6%) than in female(8.7%). Incidence of incarcerated hernia was higher in younger age, and in right side.
7) The associated congenital anomalies were found in 21.3%, and they were hydrocels(16.6%), cryptochidism(1.7%), and others.
8) Elective operation was done in 93.8% and the success rate of manual reduction in incarce rated hernia was 55.3%.
9) The diseases associated with delayed operation were noted in 16% and upper respiratory tract infection was most common disease(9.6%).
10) 88.8% of all hernia were repaired by high suture ligation of the sac and Bassini operation was done in 10.4%, Ferguson operation was done in 0.8%. the accompanied operation was comducted simultaneously in 24.7% and they were
hydrocelectomy(16.6%),
circumision(4.5%), and others.
11) The contents of hernia sac were noted in 21.0% and they were small intestine(10.7%), omentum(7.9%), and others.
12) The postoperative complications were developed in 4.8%, and they were wound absces(1.7%), scrotal swelling(1.4%), inguinal swelling(0.8%), hematoma(0.3%), and the incidence of recurrence was 0.6%).
13) Contralateral hernias were developed subseguently in 6.2% at average 1.1 years after unilateral hernia repair, and all of them were male. Incidence of subseguently developed contralateral hernia after left hernia repair was 2.1 times higher
than
after right hernia repair, and average time interval between operation and contralateral development of hernia after left hernia repair was shorter than after right hernia repair. Incidence of bilateral hernia was 11.8% including the cases who
had
bilateral hernia at the times of admission(5.6%) and developed contralateral hernia after unilateral hernia repair(6.2%).
